Instructions:

• Download, complete, and save all the necessary forms
• Be sure to have an electronic copy of your driver's license, proof of residency, and your child's latest report card
• Click on the link below for the RALC - GSPOC Documents Upload Form
• Complete the Google Form and upload ALL listed documents
• POC Early Registration - Student Information Form
• Academic & Discipline Agreement Form
• Choice Transportation Form (if transportation will be needed)
• Out of District Tuition Form (if you live out of district)
• Driver's License
• Proof of Residency address must match address on application (gas, water, electricity)
• Most Recent Report Card
